We are always learning. In healthcare, we learn most from children and families.
CC4C also learns a great deal from colleagues working across the UK and beyond. Many like-minded individuals meet at the CC4C Network. Collectively, the Network has masses of experience.
We are delighted that the Network’s experience has just turned into a publication. Lessons learnt (so far) from establishing models of integrated clinical care for children and young people is a quick guide, with tips and resources for child health leaders and for those who are developing integrated care for any age group.
